By signing this Agreement, I acknowledge that participation in golf simulator activities provided by Mulligans On Tap, a division of 509 Hype LLC, involves inherent risks including but not limited to injury from golf clubs, golf balls, simulator equipment, tripping hazards, electrical equipment, inflatable structures, falls, negligence of other participants, and other foreseeable and unforeseeable risks. I voluntarily assume all risks associated with participation. I release and hold harmless Mulligans On Tap, 509 Hype LLC, Carlos Rivas, Saturnino Hinojosa, venue owners, sponsors, contractors, employees, and affiliates from any claims arising from participation except where prohibited by law. I agree to follow staff instructions, use equipment responsibly, and accept responsibility for injuries or property damage caused by unsafe or improper conduct. I certify that I am physically capable of participating and assume responsibility for any medical expenses incurred. MEDIA RELEASE. I grant permission to Mulligans On Tap and 509 Hype LLC to use photographs, video recordings, audio recordings, and my likeness for promotional, marketing, advertising, website, social media, and sponsorship purposes without compensation. GOVERNING LAW. This Agreement shall be governed by the laws of the State of Washington. Any dispute shall be resolved in Benton County, Washington.
